Through case management model, our Cancer Adjustment and Supporting Service provides Holistic care service to cancer patients through home care nursing support by home care nurses for professional assessment and consultation. Patients can receive holistic care service while the patients are staying at home. On-site rehabilitation and nursing care as well as psycho-social spiritual support would also be provided to the cancer patients by our trans-disciplinary professional team
