This programme supports them and their families in addressing changing physical conditions and care needs. It is suitable for individuals whose physical condition is relatively stable and who require transitional, as well as long-term care.<\/span><\/span>[\/vc_column_text][vc_empty_space height=&#8221;40&#8243;][vc_custom_heading text=&#8221;Scope of Service&#8221; google_fonts=&#8221;font_family:Lato%3A100%2C100italic%2C300%2C300italic%2Cregular%2Citalic%2C700%2C700italic%2C900%2C900italic|font_style:400%20regular%3A400%3Anormal&#8221;][vc_column_text]<\/p>\n<ul>\n<li><span style=\"color: #0097d6;\"><strong>Continuous Care Support<\/strong><br \/>\n<span style=\"color: #000000;\">Facing changes in physical conditions (such as stroke, dementia, and other long-term illnesses), care needs may increase to a point where home care becomes challenging. This programme provides transitional care for patients who need long-term care services (with a maximum residential period of 5 years).<\/span><\/span><\/li>\n<li><span style=\"color: #0097d6;\"><strong>Quality Medical Care<\/strong><br \/>\n<span style=\"color: #000000;\">Resident doctors make regular rounds, working with the nursing team to provide patients with meticulous and personalized long-term care and medical support.<\/span><\/span><\/li>\n<li><span style=\"color: #0097d6;\"><strong>Integrated Psycho-Social-Spiritual Care<\/strong><br \/>\n<span style=\"color: #000000;\">Our psycho-social-spiritual team (consisting of chaplains and social workers) regularly provides pastoral care and emotional support. They also organize various social and recreational activities to enhance the quality of life for patients.<\/span><\/span><\/li>\n<li><span style=\"color: #0097d6;\"><strong>Tranquil Environment<\/strong><br \/>\n<span style=\"color: #000000;\">Our peaceful natural setting and comprehensive facilities allow residents to recuperate comfortably and tranquilly. It is used to reflect and assess the applicant\u2019s affordability. It does not necessarily reflect the total financial resources available to the applicant. For more details, please contact the social worker:<\/span><\/span><\/p>\n<ol>\n<li><span style=\"color: #000000;\"><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Household Assets Limit (including cash, savings deposits, investments and non-owner-occupied properties etc.)<\/span><\/span><\/li>\n<li><span style=\"color: #000000;\"><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">The government has published information regarding the ceiling of the household income for the past six months which cannot exceed 3.5 times the median over that period.<\/span><\/span><\/li>\n<\/ol>\n<p>[\/vc_column_text][vc_empty_space height=&#8221;40&#8243;][vc_custom_heading text=&#8221;Service Charges:&#8221; google_fonts=&#8221;font_family:Lato%3A100%2C100italic%2C300%2C300italic%2Cregular%2Citalic%2C700%2C700italic%2C900%2C900italic|font_style:400%20regular%3A400%3Anormal&#8221;][vc_column_text]<\/p>\n<ul>\n<li><span style=\"color: #000000;\"><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Applicants who meet the above criteria and pass the financial assessment are eligible for a subsidy covering 70% of the basic residential fee. The applicant is only required to pay 30% of the fee, starting from approximately HK$7,020 per month (basic rate). This fee includes rent, two routine ward rounds per month by the resident doctor, basic spiritual and social activities, meals, laundry service, basic nursing procedures, and related consumables (including incontinence care, blood glucose monitoring, simple wound care, nasogastric feeding care, oral care, stoma care, and continuous positive airway pressure (CPAP) care, etc.).<\/span><\/span><\/li>\n<li><span style=\"color: #000000;\">Other expenses are eligible for a 30% subsidy, and applicants are required to pay the remaining 70%. The actual amount depends on the level of nursing care, rehabilitation needs, and optional services chosen by the applicant.
