Admission to Inpatient Services
Haven of Hope Sister Annie Skau Holistic Care Centre provides professional medical and nursing support for individuals requiring inpatient care. The healthcare team will assess patients based on their health condition and care needs to assist in arranging suitable inpatient services.
A｜Patient receiving care in a hospital or RCHE
Please ask your doctor to complete the Referral Form. Once completed, please submit the form to us by fax 2785 0721, email [email protected], or WhatsApp 6060 2160 to apply for admission.
B｜Patient receiving care at home, or unable to provide a doctor's referral form
Patients or family members can contact our administration team via WhatsApp or phone for inquiries without submitting a referral form in advance.
If registered with eHealth, please grant sharing consent according to our instructions to allow our healthcare team to review health records for initial evaluation.
*Our healthcare team will conduct an initial assessment based on the patient’s health condition, care needs, and the information provided. Admission will be arranged subject to the assessment outcome, bed availability, and the waiting-list status. If admission is arranged, our admission nurse will contact the patient or family member(s) to confirm the admission date and time and explain the admission arrangements.
